This session will provide you with a clear understanding of the HOPE Tool, along with strategies for strengthening partnerships between hospice and skilled nursing teams to better serve residents and families.
This session is designed for both hospice providers seeking to enhance their role in skilled nursing communities and skilled nursing leaders who want to better understand how to work effectively with hospice partners. We’ll will review approaches to communication, care plan alignment, and regulatory compliance, with a focus on applying the HOPE Tool to everyday practice.
